- The Smart Choice Dental Savings Plan is a dental savings plan and is NOT dental insurance.
- Membership fees are paid in full at enrollment and are non-refundable.
- Membership benefits activate at enrollment and remain in effect for one year from the enrollment date.
- Membership benefits are only valid at Woodland Hills Family Dentistry and are non-transferable.
- The membership cannot be combined with any other dental insurance, discount plan, promotion, or reduced-fee program unless expressly authorized.
- Membership fees and benefits are subject to change at each annual renewal.
- Treatment must be paid in full at the time of service to receive membership savings.
- Membership benefits cannot be applied retroactively to treatment provided before enrollment.
- No refunds, full or partial, will be issued if a member chooses not to use some or all plan benefits.
- The plan does not cover services that are not specifically listed as included benefits.
- Woodland Hills Family Dentistry reserves the right to modify, discontinue, or replace the membership program at renewal periods.
